We cannot wait to serve you in our office! Below you will find the paperwork you can print out and bring with you on your first visit, along with what you can expect during your visit.
See you soon!
Arrival and Paperwork
Upon your arrival you will be greeted and given a chance to fill out our paperwork. If you have a printer at home, you can find our paperwork at the top of this page to fill out beforehand and bring in. Please bring your drivers license and insurance card with you. We will check your insurance benefits to see what is covered!
Tour the Office
After you fill out the paperwork, you are given a quick tour of the office to better understand who we are and the flow of our office. It is our top priority that you feel comfortable and at home in our office!
We will discuss with you one-on-one your health concerns, and the health goals you would like to achieve, along with how chiropractic care works and how we can help you reach those goals in our office. This is a great time to ask any questions you may have.
Our thorough assessment, using the most current and updated technology in the field, will evaluate the location and severity of stress and nerve interference in your body. This allows us to fully understand the state of your health, so that we can provide the best possible care that you need and deserve. Additionally, our procedures are incredibly non-invasive and involve zero pricking, poking, or prodding.
While not always needed, our office has an x-ray unit on site in case the doctors want to get a closer look at your spine, which takes about 5-10 minutes to complete. This is the last step of your first visit at our office, which allows us time to carefully review your file and assessments to ensure you are getting the care that you deserve to reach your health goals.
Following your first visit is your most important appointment in our office. It is our Doctor’s Report, where Dr. Evan and Dr. Callan go over more information on chiropractic care and healing. Following this, we will go over your individual findings, assessments, X-rays and your personal recommendations for care.
Your First Adjustment!
You finish your second visit with your first adjustment! From now on appointments in our office will take about 5-10 minutes.
